For Patients – Eccella Smiles & Aesthetics
Our patients are the most important part of our practice. We go out of our way to ensure that they have all the resources they need before, during, and after their dental visits. Informed patients make happy patients, so we’re eager to share information about our practice, as well as the latest breakthroughs in the dental world. If you have a question not answered on our website, please call our Jacksonville office. We’re passionate about dentistry and look forward to helping you better understand your specific needs.
NEW PATIENT FORMS
We know that it can be frustrating to sit down and complete paperwork when you just want your appointment to begin as quickly as possible, which is why we’ve included our new patient forms here as a download. Simply click on the link, print the forms, fill them out, and bring them to your next appointment to help things fly by. This can save you as much as 15 minutes when you come to see us! Please let us know if you’re having any issues getting this to work.
FINANCING
Comprehensive aesthetic dentistry is a wonderful investment in yourself! Nothing should stand in the way of you receiving the dental care you need – and deserve. Because insurance companies limit the amount and types of treatments they will cover, we’ve found that third-party financing can provide patients with the resources they need to move forward with treatment. Dr. Wagner works with two reputable companies, Med Loan Finance and CareCredit, so you can complete your dental treatment hassle-free. Fill out an online application and receive an instant reply, or call our office for assistance in choosing a plan that works for you. Many patients qualify for 0% interest, low monthly payments, and revolving lines of credit.
CANCELLATION POLICY
We understand that occasionally missed appointments can occur for a variety of reasons. If you are unable to keep an appointment, we ask that you cancel at least 24 hours in advance. Your appointment time is reserved especially for you, so when you miss an appointment without giving 24 hours notice, someone else who could have been seen in your place is delayed unnecessarily. Failure to give 24-hour notice of cancellation of an appointment or no-showing at an appointment may result in a charge on the patient’s account.
